Diagnostic Procedures
The Patient authorises the dentist to take x-rays, study models/scans, photographs, and other diagnostic aids deemed appropriate by the dentist to make a thorough diagnosis. The Patient authorises the dentist to perform all recommended treatment mutually agreed upon by The Patient.
Anaesthesia & Medication
The Patient agrees to the use of local anaesthetics, sedatives, nitrous oxide and other medication as necessary at the time of treatment. The Patient agrees to dental treatment recommended by the dentist(s) that they see for treatment and The Patient knows they are able to ask their treating dentist or principal dentist any questions at any time to better understand their treatment needs and options.
Questions & Communication
The Patient can ask questions about their treatment at any time. We encourage open communication to ensure you fully understand your treatment plan, options, and expected outcomes.
